No, it should not interfere with your spare. I had a 1999 Ford Ranger and then put it on a 2001 Mazda B3000. It bolts right on without any drilling. As long as it matches your model, you will have no problems

Q: Can you access the spare tire on a 02 b3000 with this?

I have no problems getting to my spare tire. but it fits in there tight. i used the hitch on my 2000 ranger. i installed my hitch with the tire in place and then removed the tire to make sure. On my ranger i have a hole about my bumper to stick the crank into to drop the tire.

Make sure the spare tire you now have under the truck is the same size as called out on the drivers side door sticker and you should not have a problem. I had a 2006 ranger and after I installed this hitch I could not get the spare back in, it was the same size as the road tired and was to big, it would fit fine before I put the hitch on. The spare on the 2006 ranger is smaller than the road tires, the Mazda may be the same.
